Bioidentical Hormone Replacement Therapy (BHRT)

Definition:

BHRT uses bioidentical hormones—molecules chemically identical to human hormones such as estradiol, estrone, estriol, progesterone, testosterone, and DHEA, usually derived from plant precursors. BHRT forms can be FDA‑approved products (e.g., estradiol patches, micronized progesterone capsules) or custom-compounded mixtures in creams, capsules, lozenges, etc.  It is the most effective medical treatment for bothersome menopause symptoms in women. Its benefits are greatest when started before age 60 or within about 10 years of menopause, in women without major contraindications.

Hormone Replacement Therapy Benefits in Women

Symptom Relief and Quality of Life

  • HRT is the gold standard for hot flushes, night sweats, and other vasomotor symptoms, and for vulvovaginal dryness/genitourinary syndrome of menopause.
  • It improves sleep problems, sexual function, mood swings, and overall health‑related quality of life, especially in symptomatic women.

Bone, Fracture, and Metabolic Health

  • HRT prevents bone loss and reduces vertebral, hip, and all osteoporotic fractures.
  • It improves lipid profile, insulin sensitivity, and components of metabolic syndrome, and can help prevent progression to diabetes in some postmenopausal women 4.

Cardiovascular and Mortality Effects

  • When started near menopause in healthy women, HRT is associated with lower coronary heart disease and reduced all‑cause mortality, with overall benefits outweighing risks in women aged ~50–60.
  • Large meta‑analyses show improved vascular function, but an increased risk of stroke and venous thromboembolism, so careful risk assessment is needed.

Cognition and Other Possible Benefits

  • Early initiation within 10 years of menopause may have neuroprotective effects on some cognitive domains and may slow cognitive decline in selected groups; evidence is positive but mixed and not definitive.
  • Some reviews report reduced risk of colorectal cancer and possibly other conditions in certain regimens

Hormone Replacement Therapy in Men

  • For men with true testosterone deficiency, bioidentical TRT can meaningfully help sexual function, muscle and bone health, anemia, and aspects of mood and energy, with a generally favorable short‑ to mid‑term safety profile under good monitoring.
  • Using testosterone purely for “anti‑aging” in otherwise healthy men with borderline levels remains more uncertain, and long‑term safety data are still limited.
  • Large trials and meta‑analyses show no clear increase in major cardiovascular events or prostate cancer in appropriately selected, monitored men over ~2–3 years.
  • Main recurring issue: erythrocytosis (high hematocrit), especially with long‑acting injections; requires blood count checks and dose/route adjustment.
  • Other concerns: possible worsening of sleep apnea, effects on fertility (suppressing sperm production), need to watch prostate‑specific antigen (PSA), and urinary symptoms.
